Spring tour participant forms (the fine print)
These forms below will help us prepare for a successful Spring Concert Tour. Please complete them by February 20, 2024.
Please make plans to join us for a Spring Tour Parent & Participant Meeting during Saturday Band on Saturday, March 2. Specific logistics and the itinerary will be released and discussed at these meetings.
We will present two meetings on March 2:
8:00 – 9:00 AM at Nolan Catholic High School in Ft. Worth
and
11:00 AM – 12:00 PM at The Shelton School in Dallas.
Student Forms
Please complete each of these forms for participants under the age of 21.
Please complete this Medical Information Form. This helps us have important information about your child in the event of an emergency at camp.
This is also where you list food allergies and other important information.
Please complete the Student Rules Form for every child attending the tour.
You can also choose your Tour T-shirt Size on this form.
Medicine Administration Kit (if student is attending unaccompanied)
If you are chaperoning the trip, you will dispense your own child’s medication.
For students attending without a parent, all medication (both prescription & over the counter) must be dispensed by our Tour Medic or designee. If your child takes medication on a regular basis, please complete this form.
Please consider the focus required to perform while on tour when making medication decisions during the trip.
Adult Forms
Please complete each of these forms if you are age 21 or older.
Please complete this Medical Information Form. This helps us have important information about you in the event of an emergency on the Spring Tour.
This is also where you list food allergies and other important information.
This form is required for all adult chaperones and participants.
